Registration Forms

NEW For 2026!

We have converted our registration forms this season to all online

We hope this is a smooth transition and is helpful for everyone. Our registration documents will utilize DocuSeal and after being signed by BOTH parent and athlete, the forms will be submitted to the Target Terminators.

Season: October through April
100 targets & ammo each month (~$500 value) – does not carry over to next month –
Reduced fees on extra practice rounds at Rio Salado Sportsman’s Club for “under 18” athletes
“ONE ON ONE” Instruction with NRA Certified Coaches
Use of SCTP Club Shotguns (No need to buy a gun!!!)

Follow the steps below to complete the enrollment forms:

Remember you will need to follow these steps for each shooter being enrolled.

STEP 1: Complete Team & State Forms
Fill out and digitally sign the Target Terminator, Arizona Fish & Game, and SSSF forms below. Because we are using DocuSeal, your signatures are securely captured online—no printing required!
Forms are still for one athlete only. You will have an option after you submit the first set of forms to fill them out again for another athlete in your family.

STEP 2: Make Your Payment
You will be directed to our payment processing page (new tab will open up in your browser). Once your payment and initial forms are securely submitted, it will alert our head administrator to start your profile in the SCTP / SSSF (Our National Organization) system.

STEP 3: Complete SSSF Dcouments (Watch your Email)
If you are enrolling a new athlete to our program, within 48 hours of completing Steps 1 and 2, check your email inbox. You will receive two separate emails from SSSF (the national organization for our SCTP shooting teams). One email will request to have the parent signature, and the second email will request the athlete’s signature.

  • This email will contain a link to fill out and digitally sign their required national forms.

STEP 4: Attend Registration & Orientation Night
Once your digital paper work is completed join us on Orientation night to kickoff the season.
