Registration Forms
PAPER FORMS ARE REQUIRED – PLEASE PRINT and BRING ALL FORMS WITH YOU TO ENROLLMENT!!!
The Registration Forms are now formatted so you can fill them out on the computer. Follow the steps below for each form.
We accept Checks / Cash / Visa / MC / Discover for registration fees ($250 for season).

Follow the steps below to complete the enrollment forms:
Remember you will need to follow these steps for each shooter being enrolled.
Complete our information sheet for each athlete as well as payments. We will then be alerted to start the process in SSSF. When we update your information in SSSF, an email will be sent to you from SSSF to complete the documents. There will be the initial email to fill in your information, then there will be separate emails from SSSF to DocuSign the documents. If the athlete is under 18, there will be 2 DocuSign emails as a separate signature for parent & athlete is required.
STEP 1: Fill out all the forms below. We have combined all the documents in one PDF document to make it easier.
STEP 2: Print out the PDF Document once you have filled it out.
PC: Right Click on the form, select Print.
MAC: Press the Ctrl (or Control) key when you tap the mouse button or the trackpad on the form and then select Print.
STEP 3: Sign all the documents and bring them to Orientation Night in September.
STEP 4: At the bottom of the form, click on the button to go to the RSTT Online Information Sheet.
DO NOT FORGET TO PRINT OUT THE FORMS: Sign the forms, bring them to Orientation Night
PC: Right Click on the form, select Print.
